Account Request
This form is the first step to enrolling your new student online. Complete it to request an account that you will use to log in to a secure system.
Complete required fields to request an account to enroll your students. An original Birth Certificate, Immunization Record and Proof of Residency (example: lease or purchase agreement, utility bill eg. water, trash, sewer, electric, gas or cable)  must be uploaded and approved by the school office staff before your student is approved for enrollment.
