Account Request
This form is the first step to enrolling your new student online. Complete it to request an account that you will use to log in to a secure system.
Complete required fields to request an account to enroll your students. An original Birth Certificate, Immunization Record and Proof of Residency must be uploaded and approved by the school office staff before your student is approved for enrollment.
If your student is registering for the 2017-2018 school year and immunizations are not yet up to date, you can send (email, fax or drop off) the updated shot record to the school before school starts in August.
